PATIENT CONSENT & AUTHORIZATION

Current as of July 2026. PepOS is a tracking and communication tool — not a medical device. It logs care a licensed provider prescribed and never sells, sources, recommends, or doses any medication.

Draft pending final legal review. This is the plain-language consent a patient accepts, opt-in and revocable, during signup. Patients never sign a Business Associate Agreement.

Before you start using PepOS, please read and agree to the following:

1. What PepOS is

PepOS is an app to track your nutrition, injections, symptoms, and how you feel, and — if you connect to a clinic — to share that with your care team. PepOS does not give medical advice and never tells you your dose.

2. Sharing with your clinic

If you enter a clinic code, you consent to PepOS sharing the information you log with that clinic's care team so they can monitor your care. You can disconnect at any time, and your clinic then stops receiving new information.

3. Not for emergencies

If you have an emergency or feel unsafe, call 911, or call or text 988. PepOS is not monitored in real time and is not for emergencies.

4. Your data

Your health data is stored on secure infrastructure built to HIPAA standards, shared only with your own clinic's care team when you connect one, and never sold. See the Privacy Policy for the full detail.

5. Not medical care

PepOS supports your care; it does not replace your provider's guidance.

6. Your consent

By continuing, you accept the PepOS Terms of Service and Privacy Policy, and you authorize PepOS to collect and process the information you enter for the purposes described above. This authorization is voluntary and you may revoke it at any time by disconnecting your clinic or deleting your account.

Your acceptance is stored with a time stamp and the document version. If the wording is updated, you will be asked to review and accept the new version.
